Collection Instructions:
Draw 1 full, pink-top (K2 EDTA) tube and send to the lab. Unique Blood Bank wristband number must be on specimen submitted to the Blood Bank. Place Blood Bank wristband on patinet's wrist.

Blood Product Information
- Indicate when PLATELET PHERESIS is to be transfused.
- Specify # of units
- Indicate on request form if special product preparation is needed (eg, irradiated).
- Appropriately labeled specimen which also includes the unique Blood Bank wristband number must be submitted to the lab.
- Consent for Transfusion of Blood and Blood Products, prior to transfusion.
